Jeremy’s Story

At 22 years old, Jeremy found himself searching for a sense of purpose and direction. He decided to join the Navy and proudly served our country for more than two decades. Jeremy’s esteemed military career took him across the globe and earned him a well-deserved retirement. However, after leaving the service, he faced an uphill battle. The mental scars of PTSD made it difficult for him to access the resources he desperately needed for a stable life. Ultimately, Jeremy found himself living out of his car with his beloved cat, Baby Girl.

As winter settles in, too many people, like Jeremy, face the cold nights living unsheltered. Our mission is to help people transition from homelessness to housing — and with your help, we can provide vital resources for our most vulnerable neighbors, so they have a roof over their head. Throughout the past year, our efforts have continued in three critically important areas:

  • Outreach & Support — Our Navigation Team connects with people living outside to work on next steps to housing, while our Resource Center helps over 300 people daily meet their basic needs.

  • Shelter Services — On any given night, more than 700 individuals and couples (plus their pets!) have a safe place to sleep, connect to services, and work towards permanent housing.

  • Housing — Our team works year-round to find permanent solutions for people who are experiencing homelessness, placing nearly 900 people into affordable housing last year.

This year we helped over 9,000 people, each with a unique story like Navy Veteran Jeremy.

After months of experiencing homelessness, the Department of Veterans Affairs connected Jeremy with Transition Projects. His Veterans Services case manager, Lauren, helped Jeremy in relieving the barriers he was experiencing while guiding and supporting the steps he took to find affordable and stable housing.

Since I’ve been in the program, I’ve been able to focus on my mental health … and that freedom that I’m allowed with support from Transition Projects is a blessing.
