Permanent Supportive Housing Pre-Screening Form

Permanent Supportive Housing (PSH) is long-term housing to help individuals and families who are experiencing chronic homelessness (more than one year) and those with disabilities. This type of housing combines affordable living spaces with access to support services, such as healthcare, mental health counseling, substance abuse treatment, and job training. The goal of PSH is to provide stability and support, enabling participants to live more independently and improve their quality of life. This type of housing addresses both housing needs and the underlying issues that contribute to homelessness.

Pre-screening Criteria: If you currently reside in Multnomah County, are chronically homeless (more than one year), and suffer from a disabling condition, please complete the form below.

Someone from our Coordinated Housing Access Team will follow up with you if you meet the full eligibility criteria.

NOTE: Completing this form does not automatically qualify you for permanent supportive housing.
